5.3.1 Antihistamines

Older first-generation sedating antihistamines tend to worsen RLS symptoms. This includes many drugs added to combination with cold remedies and OTC sleeping pills (e.g., diphenhydramine or doxylamine). Newer second-generation nonsedating antihistamines (e.g., loratadine, desloratadine, fexofenadine) generally do not worsen RLS. For treating rhinitis in patients with RLS, safe alternatives to using antihistamines include corticosteroid- and cromolyn-based nasal sprays and montelukast (an anti-inflammatory).
